Our social support services for adults and seniors are person-centered and designed to empower individuals to adopt a healthier lifestyle while staying mentally active and socially connected. They consist of targeted interventions that help older adults enhance their well-being, improve their quality of life, and maintain independence for longer, primarily from the safety and comfort of their own home.

All activities and services aim at promoting a Sense of Purpose and Control by:

  • Respecting individual preferences and choices: Allowing adult clients to make decisions about their care, activities, and daily routines. 
  • Encouraging meaningful activities: Supporting participation in activities that provide a sense of accomplishment, purpose and joy, while promoting physical wealth and challenging the mind. 
  • Creating opportunities for social interaction: Provide companion and facilitate connections with friends, family, and the wider community (incl. through volunteering).

Support and Empowerment

We provide structured support and organize a diverse range of activities—both in-home and outdoors—that align with your interests and personal goals, while helping you stay healthy and independent.

To ensure the best fit, we conduct a thorough background analysis of each client’s profile through an initial assessment during a discovery call or meeting. The type, frequency, duration, and timing of services are tailored to each person’s needs and aspirations, based on the outcomes of this collaborative assessment.

How it works

First, we design a personalized social care plan in collaboration with the client. The specific types and scope of services are selected according to the client’s profile, desires, and needs, while also considering any practical constraints or limitations.

The services that we provide are grouped into three main categories:

  • Cognitive stimulation
  • Physical activity
  • Emotional well-being

All activities are carried out with the full agreement of the client and, when appropriate, their family. We can modify or terminate them at any time based on the client’s wishes.

    Assessment of client needs

    We conduct a careful evaluation of each client’s individual needs, which forms the basis for a tailored care and support plan. This stage involves close collaboration with family members, household members, or other significant people to gain a deep understanding of the client’s preferences, desires, as well as any restrictions or special requirements.

  • Stage 2

    Development of a personalized care & support plan

    A customized social care and support plan is designed in alignment with the findings of the assessment phase. This plan is co-created with the client and, when appropriate, their family or significant others. The client’s wishes are respected throughout the process, as they are the primary recipient of the services.

    Implementation of primarily home-based social care and support services that promote the well-being of adults and seniors wishing to live independently for longer. Our approach is holistic, addressing physical health, cognitive stimulation, emotional well-being, social engagement, and a safe and supportive environment.     We start with a 20–30 minute discovery call or meeting to explore your needs, goals, and desired outcomes.

  • Step 2: Planning

    Together, we co-create a personalized care plan that aligns with your needs, aspirations, schedule, and daily commitments.

  • Step 3: Commitment & Delivery

    We organize a set of planned activities, delivered at your home or in safe, pre-arranged settings (e.g., outdoor outings).

  • Step 4: Follow-up

    We continuously monitor the plan, making adjustments as needed based on your feedback and, if relevant, your family’s input.

With long-standing experience in delivering social support services to diverse at-risk groups, I provide personalised, practical, and evidence-based interventions that respect each person’s dignity, uniqueness, and daily reality.

Over the years, I have supported older adults, people with disabilities, migrants, survivors of violence, and many others — learning that small, consistent steps can profoundly transform well-being.

My work focuses on psychosocial support, well-being promotion, mentoring, and tailored guidance. I follow a holistic approach to healthy ageing, combining physical activity, balanced nutrition, emotional well-being, cognitive stimulation, and meaningful social connection.

Drawing on my background as a social work and policy expert, I now offer personalised, home-based programs that help adults — and especially older adults — maintain independence, confidence, health, and quality of life. My goal is to help every individual live a healthier, more connected, and more fulfilling everyday life.
